Please Note:

• Registration is in person at the Administrative Office or by may.  Call to check availability at (815) 459-0680.
• A non-refundable deposit of the first month's tuition is required.
• To be added to a waiting list, a non-refundable deposit of $25 is required for each classs.
• Tuition is discounted 5% for second and third children of same immediate family.
• Please determine your child’s placement based on their age on September 1.
• Payment of one month’s tuition is required at time of registration.
• Wait list registration requires a deposit payment of $25.00.
• Complete one registration form for each child and class code number.

Starts Tuesday, September 7 or Wednesday, September 8, 2010

After February 1, register by mail, in person, or by fax.

Preschool Fees (3 and 4 year old class)
Program Fee for 2 Days: $159/month
Resident Discount Fee for 2 Days: $144/month
Program Fee for 3 Days: $202/month
Resident Discount Fee for 3 Days: $187/month

4-5 Years
This is a four day class for children who will be five years old (before December 31) or who are eligible, but not attending kindergarten.

Overdue Bill Policy

It is the policy of the Crystal Lake Park District that any Extended Time or Preschool account 30 days past due, be notified that if the balance, including the current month, is not paid in full by the 15th, the participant will not be allowed to attend until the balance is paid in full.
